I saw question where it was mentioned that PM is ic creating wbs and he goes to OPA and find another project template and used to to create for his project. Which of the following is tool of the create wbs process is he using?

Two choices to consider are Decomposition and Template.
The answer was template. I am partially unconvinced with the logic. If they are asking tool then it should not be input-template as asnwer.

Well yeah, if you go by PMBOK meaning of the word 'tool', then Template is not a tool for Create WBS. But in the context of the question, Template is coming from OPA, which is an input, and the PM is using a Template.

Anyway, I don't like the question in the first place if it tells you that PM is using a Template, and then asking what he's using.

The only tool in create wbs is Decomposition.
